Community Health Needs Assessment

In late 2021, Salinas Valley Health joined five other local health organizations to form the Monterey County Health Needs Collaborative, a partnership established to conduct a comprehensive community health needs assessment (CHNA) for Monterey County.

Months of primary and secondary data collection are contained in the assessment including extensive input from the community and experts in public health, clinical care, and others. This CHNA process and report are presented in compliance with current federal requirements and will serve as a basis for implementing strategies by the Healthcare System and Collaborative partners to address these community needs.

Individuals ratings for each criteria were averaged for each tested health issue and these composite criteria scores were averaged to produce an overall score. This process yielded the following prioritized list of community health needs:

  • Diabetes
  • Mental Health
  • Access to Health Care Services
  • Nutrition, Physical Activity & Weight
  • Heart Disease & Stroke
  • Substance Use
  • Housing
  • Infant Health & Family Planning
  • Injury & Violence
  • Cancer
  • Oral Health
  • Potentially Disabling Conditions
