Taylor Farms Family Health and Wellness Center


South Monterey County is home to thousands of individuals and families who have depended on care provided by Gonzales Medical Group (GMG) since the 1930s and Christine Ponzio, MD, FAAFP who joined GMG in 1984. Preventive care and active management of conditions such as diabetes, asthma, osteoporosis, obesity and heart disease is critical to the health of this community.

When Salinas Valley Memorial Hospital opened its doors in 1953, the founders dedicated the hospital “to the health of the community,” a commitment that continues to this day. As a outgrowth of our shared mission to make excellent healthcare services available to more people throughout the region, Salinas Valley Memorial Healthcare System has teamed up with Dr. Ponzio to establish the Taylor Farms Family Health & Wellness Center in Gonzales.

With a focus on prevention, wellness and disease management, the Taylor Farms Family Health & Wellness Center is ushering in a new era of care in South County communities. The Center is focused on vital individual and family medicine including:

• Annual, sports and school physicals
• Laboratory services, including
• Asthma care testing results onsite
• Diabetes screening, diagnosis and management
• Wellness programs and patient
• Immunizations education

To best serve the residents of South Monterey County, we have built a state-of-the-art, 6,400-square-foot medical facility which opened in November of 2015. With larger, up-to-date facilities, and the addition of two bilingual physicians, Dr. Ponzio’s practice is able to increase its patient care capacity from 17,000 to more than 25,000 visits a year.

Taylor Farms Family Health & Wellness Center was formed as a hospital-based Rural Health Clinic (RHC). The purpose of the RHC program is to improve access to primary care in underserved areas with the goal of improving health and quality of life for individuals and families. RHC’s are certified to receive special Medicare and Medicaid reimbursement.





Giving and naming opportunities are available

 
You have an opportunity to play an important role in bringing vital healthcare services to more people in South Monterey County. Through a capital campaign, our goal is to raise $3 million from donations, including those that offer naming opportunities. In May, Bruce Taylor, Chairman and CEO of Taylor Farms announced the company was generously donating one million dollars to the project. In addition to Taylor Farms, significant contributions have come from American Farms, The Sally Hughes Church Foundation, Mike and Mary Orradre, Constellation Brands, D'Arrigo Bros. Co., of California and The Andrew & Phyllis D'Arrigo Charitable Foundation, Boggiatto Produce, The D'Arrigo-Martin Family, The Haynes Charitable Foundation and Cinderella Carpet, The Nunes Company, Inc., RC Farms, Dennis and Janice Caprara, Smith and Lindley, J. J. & H. Violini and The Vosti Family. Naming opportunities are still available and a donor wall is located in the lobby.
