The Heart Failure Program (CHF Clinic) is designed as an outpatient clinic, working closely with SVMH, emergency departments, primary care physicians, cardiologists and home health agencies. The clinics take a proactive, multi-disciplinary team approach to the prevention and treatment of congestive heart failure.
Since hospital and emergency department discharges for congestive heart failure require close follow-up, the clinic will have same day or next day appointments for urgent or semi-urgent appointments.
Congestive Heart Failure: What is it?
Heart failure occurs when your heart is weakened and unable to pump effectively. The inability of the heart to squeeze effectively or relax appropriately, causes blood to back-up in the lungs. Increased pressure forces fluid from the blood into the breathing spaces of the lungs and elsewhere. This causes shortness of breath, unwanted swelling and decreased oxygenation of your blood.
