Screenings
The first step to gathering baseline health data for both the individual/employee and the organization
Confidential, HIPAA compliant
- Administered onsite
- Employees receive results within 10 minutes of completing the procedure
- Registered Nurses (RN) will provide participants with results specific feedback, recommendations, and education
Includes:
- Lipid Panel- Total Cholesterol, LDL, HDL, TC/HDL, Triglycerides, Glucose
- Blood pressure
- Body Mass Index (BMI)
- Hemoglobin A1C
Health Risk Appraisal (HRA)
Analyzes biometric data and health behaviors to assess the overall health risks of the individual/employee
- Each employee will receive a customized report with their results
- Aggregate data captured from the HRA and biometric screenings will be provided to the employer to identify workforce health strengths and weaknesses
- Based on this data, appropriate interventions for the organization can be determined
Level II
Health Education
Targeted programs and activities designed to improve the health status of employees
- Customized, interactive sessions with a health professional on health prevention and disease management
- Scheduled onsite, at convenience of participants
Level III
Health Coaching Program
Designed to reduce risk factors in employees with chronic health conditions, managed by a team of health professionals
- Enhances primary care provider's plan of care
- Employees begin the program with goal setting, to promote active participation in the lifestyle modification process
- Sessions occur on-site or in the home, frequency determined by organization
Biometric tracking available to monitor progress reaching health goals
- Employer establishes criteria for entry and incentives awarded for participation
- Outcome/ROI reporting available to employer
