Medical Insurance No cost for employee. Dependent coverage available with pre-tax premiums. 100% coverage for In-Patient Services provided at SVMH; Out-Patient Services at Salinas Urgent Care and Doctors on Duty. 80/20% for Major Medical fees (Doctor/Professional fees). $100 calendar year deductible.
Dental Insurance No cost for employee. Dependent coverage available. See schedule of benefits. Orthodontics available after 1 year on Dental Insurance plan.
Vision Insurance No cost for employee. Dependent coverage available. See schedule of benefits.
Prescription Drugs 80/20% Plan: Employee pays 20% for medication from participating pharmacies.
Flexible Benefit Plan (AFLAC) Opportunity to participate in the following AFLAC programs: Unreimbursed Medical Expenses (Pre-Tax); Child/Dependent Care Expenses (Pre-Tax); Accident/Sickness/Disability Insurance; Hospital Intensive Care Protection/Organ Transplant Benefit Rider; Personal Short-Term Disability Income Protection Insurance & Premium Cancer Expense Protection Insurance (some are pre-tax and others are post-tax).
Long Term Disability 60% of pre-disability earnings after 6 months of disability (**Local 250, CNA, & Non-Affiliated).
Life/AD&D Insurance ½ x annual salary for first 5 years continuous employment ($40K maximum); 1 x annual salary for 5+ years continuous employment ($40K maximum).
