Level I – The first step to a healthier workforce

Biometric screenings (no food or drink 12 hours prior) include:

  • Lipid Panel—Total cholesterol (LDL, HDL, TC/HDL), triglycerides and glucose
  • Blood pressure
  • Body Mass Index (a measure of body fat based on height and weight)
  • Waist Circumference and current weight
  • Hemoglobin A1c (used to determine blood sugar controls for people with diabetes)

Health Risk Appraisal (HRA) 

  • The biometric data as well as lifestyle information gathered through the HRA is used to create an individual report for each participant
  • Registered Nurses (RN) are onsite to discuss screening results and offer specific feedback, recommendations and education options available

Level II – Take your wellness [at] work program to the next level

  • Features targeted programs and activities designed to improve the health status of your employees
  • Your customized program encompasses access to SVMHS Health Promotion resources as well as onsite or community-wide, interactive sessions and group presentations by health professionals on prevention and disease management
  • If you choose, it may also include ongoing program development, implementation and evaluation, and enhancement of existing wellness activities

Level III – Continue on your path to workplace wellness with our Health Coaching Program

  • Designed to reduce risk factors in employees with chronic health conditions
  • Managed by a team of health professionals
  • Supports the primary care physician’s plan of care
  • Employees begin the program with goal setting to promote success in lifestyle modification
  • Sessions can take place onsite or in the employee’s home
  • You determine the frequency of coaching sessions for each employee
  • Biometric tracking is available to monitor progress in reaching health goals
  • You establish criteria for entry and incentives awarded for participation
  • We provide outcome/Return on Investment reporting to track the effectiveness of your program
